Abstract

Background: Diabetic neuropathic pain (DNP) is a chronic and painful complication of diabetes mellitus that is often difficult to treat. Perineural platelet-rich plasma (PRP) injection has the potential to enhance vascular endothelial growth factor, tissue remodeling, and axonal nerve regeneration.Objective: To evaluate the effectiveness of perineural PRP injection in reducing pain and neuropathy severity in DNP patients at Dr. Mohammad Hoesin Hospital Palembang.Methods: A single-blind, add-on randomized controlled trial (RCT) involving 30 subjects divided into PRP and placebo (0.9% NaCl) groups. Assessments were conducted at weeks 0, 2, 4, 6, and 8 using the Numeric Pain Rating Scale (NPRS) and Toronto Clinical Neuropathy Score (TCNS).Results: The PRP group showed a significant NPRS reduction from 7.26 ± 1.75 to 3.73 ± 1.86 (p < 0.001), while the placebo group decreased from 7 (3–9) to 5 (3–8) (p = 0.027). PRP had a greater effect on NPRS reduction (48.6%) with significant differences at weeks 4 (p = 0.029), 6 (p = 0.005), and 8 (p = 0.010). TCNS decreased significantly in both groups (PRP: p < 0.001; placebo: p = 0.003), but no weekly difference was found between groups.Conclusion: Perineural PRP injection is effective in reducing moderate to severe pain intensity but not effective in improving neuropathy grade.
