Abstract

Background: Placenta accreta spectrum disorder (PASD) is a serious obstetric complication that can lead to massive hemorrhage and maternal death. Definitive diagnosis is typically established intraoperative and histopathological findings, making early detection essential. The tramline sign on 3D ultrasound has emerged as a promising non-invasive diagnostic tool; however, scientific evidence regarding its accuracy, particularly in the Indonesian population, remains limited. This study aimed to evaluate the sensitivity and specificity of the tramline sign compared to histopathological findings in predicting PASD. Method: Cross-sectional diagnostic study conducted at Dr. Mohammad Hoesin General Hospital, Palembang, from January to December 2024. The study included 50 pregnant women diagnosed with anterior placenta previa who met the inclusion criteria. A 3D ultrasound was performed to detect the tramline sign, and diagnoses were confirmed by postoperative histopathological examination. Result: Incidence of PASD in 2024 was 6.83% among all cesarean deliveries and 5.02% of total births. Among 50 patients, 40 (80%) were confirmed to have PASD based on AP findings. A positive tramline sign was observed in 42 patients, 38 of whom (90.5%) had PASD. Among eight patients with a negative tramline sign, six did not have PASD (75%). Diagnostic performance showed a sensitivity of 95%, specificity of 60%, positive predictive value of 90.48%, negative predictive value of 75%, positive likelihood ratio of 2.37, negative likelihood ratio of 0.08, and overall accuracy of 88%. Conclusion : Tramline sign on 3D ultrasound demonstrates high accuracy and remarkable sensitivity in detecting PASD. Despite moderate specificity, it has the potential to serve as a valuable non-invasive screening modality in managing high-risk pregnancies.
