Abstract

CORRELATION BETWEEN TUMOR-INFILTRATING LYMPHOCYTES CD8 DENSITY AND CERVICAL CANCER STAGE AT DR. MOHAMMAD HOESIN GENERAL HOSPITAL PALEMBANG Background: Cervical cancer is the fourth most common cancer among women worldwide, with Human Papillomavirus (HPV) infection recognized as its primary etiological factor. The immune response to HPV infection plays a critical role in its pathogenesis, with Tumor-Infiltrating Lymphocytes (TILs), particularly CD8 T cells, involved in tumor cell elimination. Although the role of CD8 TILs has been widely studied, local data regarding their correlation with cervical cancer staging remains limited. Methods: This study was an analytical observational case-series study involving 27 patients with cervical cancer at Dr. Mohammad Hoesin General Hospital, Palembang. CD8 TIL density was assessed using immunohistochemical staining and correlated with cancer stage based on the FIGO classification. Statistical analyses included Kendall’s tau-b correlation test and ROC curve analysis to determine the optimal cut-off value. Results: The majority of patients were diagnosed at an advanced stage (FIGO IIB–IVB). There was no significant correlation between CD8+ TIL density and cancer stage (r = – 0.020; p = 0.889). A cut-off value of 14.4 yielded a sensitivity of 28.6% and specificity of 80%, while a cut-off of 27.4 demonstrated a sensitivity of 95% and specificity of 14.3%. The area under the ROC curve (AUC) was 0.511 (95% CI: 0.240–0.781; p = 0.934). Conclusion: There was no significant correlation between CD8 TIL density and cervical cancer stage. However, the 14.4 cut-off point may have potential diagnostic value for identifying early-stage cervical cancer. Key word: Cervical cancer, CD8+ TILs, immunohistochemistry, FIGO stage, immunologic biomarker
