Abstract

ABSTRAK Latar Belakang Karsinoma sel basal (KSB) merupakan kanker kulit non melanotik tersering dijumpai di dunia. Umumnya lokasi KSB dijumpai pada regio kepala, wajah dan leher dengan rentang usia 40-70 tahun. Peran PD-1/PDL-1 pada KSB telah banyak diteliti terutama pada KSB stadium lanjut lokal, inoperable dan metastasis dimana hasil penelitian ini menunjukan ekspresi PD-1/PDL-1 yang tinggi pada jenis KSB tersebut. Namun penelitian yang khusus membahas PD-1 pada KSB berdasarkan risiko kekambuhan belum ditemukan. Metode Penelitian ini merupakan penelitian uji korelasi dengan pendekatan cross-sectional pada 35 sampel KSB periode 1 januari 2019 hingga 31 desember 2022. Pulasan imunohistokimia menggunakan antibodi monoclonal PD-1 klon NAT105 (Abcam). Analisa antar variabel memakai uji Chi-square dan Somers’d. Hasil Karsinoma sel basal rata-rata dijumpai pada usia 58 tahun dengan rentang 22 hingga 84 tahun dan lokasi tersering pada area kepala leher. Korelasi ekspresi PD-1 dengan subtipe risiko kekambuhan KSB didapatkan arah korelasi yang negatif dan signifikan (p=0,000) dengan kekuatan koefisien korelasi sedang (r=-0,538), namun tidak dijumpai korelasi yang bermakna antara ekspresi PD-1 dengan usia (p=0,435) maupun dengan lokasi tumor (p=1,000). Kesimpulan Karsinoma sel basal risiko kekambuhan rendah menunjukkan ekspresi PD-1 tinggi, sebaliknya KSB risiko kekambuhan tinggi menunjukkan ekspresi PD-1 rendah. Perlunya pertimbangan kembali untuk pemberian terapi anti PD-1 pada KSB subtipe kekambuhan risiko tingi. Kata Kunci: Karsinoma sel basal, PD-1, risiko kekambuhan tinggi, risiko kekambuhan rendah
