Abstract
Background: Papillary thyroid carcinoma (PTC) commonly harbors theiBRAF V600Eimutation. This mutation is associatediwith tumor aggressivenessiand poor clinical outcomes, but its detection through DNA sequencing and immunohistochemistry (IHK) is often not always available in all health facilities besides the cost is relatively high. An easier and more widely applicable approach is necessary to assess the probability of BRAF V600E mutation, especially through histomorphological parameter analysis. Several parameters have been shown to have predictive value although thereiiare still a numberiiof gaps in previous studies. Theiithree parameters that are relatively easy to identify in daily practice are histological variants, nuclear scores, and stromal reactions. The use of these three parameters as diagnostic predictors remains limited. This studyiaims to explore the relationshipibetween histological variants, nuclear scores, and stromal reactions with BRAF V600E expression as an alternativeiapproach to predict the risk of this mutation. Methods: A cross-sectional analytical descriptive study using archives of PTC surgical specimens that had been histopathologically analyzed during theiperiod of Januaryi1, 2020-Junei30, 2024. A total of 32 samples wereiselected byinon-random samplingiand divided based on histological variants. Clinicopathological data were obtained from medical records. BRAF VE1 IHK examination was performed to determine BRAF V600E expression. Univariate,iibivariate andiimultivariate analyses wereiiperformed with backward logisticiiregression to identify predictive factors for BRAFiV600E mutations. Results: Bivariate analysisishowed that parameters associated with BRAFiV600E expression were classic-tall cell histological variant (p<0.001),inuclear score 3 (p=0.003),iiand stromal reaction (p=0.027). Multivariate analysis showed that histological parameters associated with BRAF V600E expression were classic histology variant-tall cell (p=0.005, OR 42.5) and stromal reaction (p=0.081, OR 8.2) which could predict BRAF V600E expression. Conclusion: BRAFiiV600E expression wasiimore frequently found in PTC with classici histology variant or tall cell, nuclear score 3, and accompanied by stromal reaction. Thereiiwas a statistically significant association between BRAFiiV600E expressioniand PTC with classic histology variant or tall cell and accompanied by stromal reaction.
