Abstract
Introduction: Epilepsy is a manifestation of brain disorders that is associated with high morbidity. Decreased vitamin D levels and incidence of metabolic syndrome in patients receiving antiseizure medications (ASMs) have been reported. This study aimed to analyse the effect of vitamin D administration on changes in plasma levels of HOMA-IR, adiponectin, homocysteine, and hs-CRP as biomarkers of metabolic syndrome and atherosclerosis in epilepsy patients receiving enzymatic AEDs. Methods: This study encompassed a double-blind placebo-controlled randomised controlled trial (RCT) design. The intervention was vitamin D dose of 2,000 IU per day for 12 weeks. Measurements of vitamin D, HOMA-IR, adiponectin, hs-CRP, and homocysteine levels were conducted before and after the intervention and differences in levels were considered significantly different when statistical tests showed p<0.05. Results: 40 patients were enrolled at baseline, of whom 34 completed the study and were allocated in a 1:1 ratio to both groups. Vitamin D, HOMA-IR, adiponectin, homocysteine, and hs-CRP levels before intervention did not differ between the two groups. After the intervention, there was a significant increase in mean 25(OH)D levels in the vitamin D group compared to placebo. There were significant differences in changes in 25(OH)D, adiponectin, and hs-CRP levels (p<0.05), where there was an increase in 25(OH)D and adiponectin levels with a decrease in hs-CRP levels. Conclusion: Vitamin D administration in epilepsy patients receiving enzymatic AEDs has a significant effect on the increase of vitamin D levels as well as changes in adiponectin and hs-CRP biomarker levels as biomarkers of metabolic syndrome and atherosclerosis.
