Abstract

Background: Cytomegalovirus retinitis (CMVR) is the most common opportunistic ocular infection and a leading cause of blindness related to HIV- AIDS. CMVR frequently occurs in patients with low CD4 counts, particularly below 50 cells/μL. Epidemiological studies on the correlation between CD4 levels and CMVR incidence remain limited, especially in Indonesia. Objective: This study aims to analyze the accuracy of CD4 levels in predicting CMVR in HIVAIDS patients at RSUP Dr. Mohammad Hoesin Palembang. Methods: This diagnostic study employed a prospective case-control approach. The sample consisted of 51 HIV-AIDS patients examined at the Ophthalmology Clinic, VCT Clinic, Internal Medicine Clinic, and Inpatient Unit of RSUP Dr. Mohammad Hoesin Palembang from January 2024 to December 2024. Data analysis was conducted using the chi-square test, ROC analysis to determine the CD4 cut-off point, as well as sensitivity and specificity tests. Results: The analysis revealed a significant correlation between CD4 levels and CMVR incidence (p<0.05). The optimal CD4 cut-off point for predicting CMVR was found at <32,5 cells/μL, with a sensitivity of 88,2 % and specificity of 91,2%. The positive and negative predictive values also demonstrated high accuracy in diagnosing CMVR in HIV-AIDS patients. Conclusion: A CD4 count of < 32,5 cells/μL has high accuracy in predicting CMVR in HIVAIDS patients. Therefore, CD 4 monitoring can serve as an important clinical indicator for detecting CMVR risk, allowing early intervention to prevent severe complications.
