Abstract

Background: HIV/AIDS remains a global health issue with high morbidity and mortality. One of its common complications is hypogonadism, characterized by decreased serum testosterone due to hypothalamic-pituitary axis disruption. It can impair quality of life and contribute to metabolic issues like muscle weakness, depression, osteoporosis, and cardiovascular disease. Hypogonadism has been reported in both naive HIV-infected men and those receiving antiretroviral therapy (ART). This study aimed to assess the correlation between serum testosterone levels and CD4 T lymphocyte counts in naive HIV-infected men, to evaluate the impact of immunological impairment on hormonal function at diagnosis. Methods: This was an analytical observational study with a cross-sectional design, conducted at Mohammad Hoesin Hospital, Palembang, from April to October 2024. A total of 64 naive HIV-infected men were enrolled. Data were obtained through measurement of total serum testosterone levels, CD4 T cell counts, and assessment of hypogonadism-related symptoms. Results: The mean age was 36.52 ± 11.03 years, and mean BMI of 19.52 ± 3.71 kg/m². Most subject (62.5%) reported men who have sex with men. Mean serum testosterone level was 446.75 ± 260.95 ng/dL, median CD4 count was 23.5 cells/µL. Poor sleep quality was observed in 81% of subjects, mild depression in 38.1%, and mild erectile dysfunction in 28.6%. Hypogonadism occurred in 32.8% of subjects (28.1% secondary and 4.7% primary). A weak but statistically significant positive correlation was found between serum testosterone levels and CD4 counts (r = 0.313; p = 0.012). No significant difference in testosterone was found across HIV clinical stages (p = 0.464). Conclusion: There is a significant positive correlation between serum testosterone levels and CD4 T lymphocyte counts in naive HIV-infected men. Hormonal disturbances may arise early in infection, supporting the need for testosterone assessment during initial evaluation. Keywords: HIV, testosterone, CD4, hypogonadism
