Abstract

Background: The progression of chronic Hepatitis B Virus (HBV) infection depends on immune mechanisms, which classify it into four phases. Both nonspecific and specific immunity, such as interferon gamma (IFN-γ) and CD8+ T cells, play roles in eliminating HBV infection during these phases. This study aimed to analyze the relationship between IFN-γ levels and CD8+ T cell counts across different phases of chronic HBV infection. Methods: This analytical observational comparative study was conducted at Mohammad Hoesin Hospital Palembang from August to November 2024. The subjects were chronic hepatitis B patients who had not received antiviral therapy, selected based on inclusion and exclusion criteria. IFN-γ levels were measured using the ELISA method, while CD8+ T cell counts were analyzed using flow cytometry. Data were analyzed using SPSS version 29. Results: A total of 38 chronic hepatitis B patients meeting the inclusion criteria participated in this study. The reactivation phase showed the highest IFN-γ levels (0.98±0.35 pg/mL), while the inactive carrier phase had the lowest levels (0.40±0.30 pg/mL). The highest CD8+ T cell count was found in the inactive carrier phase (710.22±196.08 cells/μL), and the lowest was in the immune active phase (415.5±304.02 cells/μL). The differences between phases were statistically significant (p<0.05, one-way ANOVA). Age in HBeAg-positive patients was a significant independent predictor for the immune active phase, whereas IFN-γ levels were the only predictive factor for the reactivation phase in HBeAg-negative patients. Conclusion: Serum IFN-γ levels and CD8+ T cell counts are associated with the different phases of chronic hepatitis B infection.
