Abstract

Background: Aging leads to a progressive decline in muscle mass and strength, which contributes to reduced physical function in older adults. Malnutrition, particularly protein deficiency, is a major factor accelerating this decline. Whey protein is a high-quality protein rich in essential amino acids, especially branched-chain amino acids (BCAAs), and has the potential to improve serum albumin levels and muscle strength in the elderly. This study aimed to evaluate the effect of whey protein powder supplementation on serum albumin levels and handgrip strength in geriatric patients. Methods: This was an experimental, open-label clinical trial involving patients aged >60 years who met the inclusion and exclusion criteria. The intervention group received whey protein supplementation for 30 days. Serum albumin levels and handgrip strength were measured before and after the intervention. Data were analyzed using bivariate and multivariate statistical tests. Results: Forty geriatric patients (mean age 70.07 ± 5.40 years; 72.5% female) participated, with 65% at risk of malnutrition despite an average BMI within the normal–obese range. All participants had normal cognitive and mental status, and 90% reported high physical activity levels. After whey protein supplementation, serum albumin increased slightly (4.24 ± 0.31 to 4.27 ± 0.29 g/dL; p = 0.001), while handgrip strength improved significantly (25.22 ± 6.64 to 26.42 ± 7.04 kg; p = 0.002), albumin were not influenced by demographic or clinical factors, whereas handgrip strength improvement was positively predicted by better nutritional status (p = 0.028) and negatively by higher baseline physical activity (p = 0.026). Conclusion: Whey protein powder supplementation had a positive impact on handgrip strength but only a minimal effect on serum albumin levels in older adults. This intervention may serve as a nutritional strategy to improve functional status in geriatric care. Keywords: elderly, whey protein, albumin, muscle strength, handgrip, malnutrition
