Abstract

ABSTRACT THE RELATIONSHIP BETWEEN FERRITIN LEVELS AND PREGNANCY-ASSOCIATED PLASMA PROTEIN A (PAPP-A) IN PLACENTA ACCRETA: A CASE STUDY USING IMMUNOHISTOCHEMICAL EXAMINATION Background: Placenta accreta (SPA) is a major cause of obstetric hemorrhage with high complication risks. Identifying predictive biomarkers such as ferritin and Pregnancy-Associated Plasma Protein A (PAPP-A) may assist early detection and optimal management. Methods: This observational case-control study was conducted from June 2024 to June 2025 at the Outpatient Clinic, Emergency Department, and Obstetrics Ward of Dr. Mohammad Hoesin General Hospital Palembang. The study involved 36 third-trimester pregnant women (18 with SPA and 18 without SPA). Maternal serum ferritin was measured, and placental PAPP-A expression was evaluated using immunohistochemistry. Statistical analyses included chi-square, Mann-Whitney U, ROC curve, and logistic regression. Results: Serum ferritin was significantly lower in PAS group (32.8 [17.5-173.1] ng/mL vs. 77.3 [31.1-193.2] ng/mL; p = 0.003); low serum ferritin increased PAS risk (OR: 5.50; 95%CI: 1.06-31.53). PAPP-A expression was higher in PAS (10.83 ± 1.72 vs. 6.22 ± 3.34; p < 0.001) and PAPP-A expression ≥ 9 increased PAS risk (OR: 13.00; 95%CI: 2.12-93.55). Combined abnormal ferritin and PAPP-A increased PAS by 12.57 (95%CI: 1.84-134.73) times. Abnormal PAPP-A is more significant to PAS than ferritin. Conclusion: Low ferritin and high PAPP-A expression are significantly associated with SPA. Keywords: Placenta accreta, ferritin, PAPP-A, immunohistochemistry, biomarker, third trimester pregnancy
