Abstract

Background: After functional endoscopic sinus surgery (FESS), nasal packing is placed for up to 48 hours to prevent postoperative bleeding, hematoma, synechia, and infection. Postoperative pain after FESS arises from mucosal wounds and inflammation, and is exacerbated by pressure from the nasal tampon on the injured mucosa. Levobupivacaine is a topical anesthetic that can reduce local pain following FESS. This study aimed to evaluate the effectiveness of 0.25% levobupivacaine applied to nasal tampons in managing postoperative pain after FESS. Methods: A quasi-experimental study was conducted at Mohammad Hoesin Hospital Palembang from April to June 2025, with 32 eligible patients. Levobupivacaine 0.25% was applied to nasal tampons, and pain was evaluated using the Visual Analog Scale (VAS) at 4, 8, 12, 24 hours postoperatively and at the time of tampon removal. Mucosal bleeding scores during tampon removal were assessed by nasoendoscopy. Results : Postoperative pain after FESS was mild but increased to moderate during nasal tampon removal. Levobupivacaine 0.25% provided a significantly greater analgesic effect compared to normal saline, especially at 8 hours (VAS 8.75 vs 20.63; p = 0.000) and 12 hours (VAS 11.25 vs 24.38; p = 0.000) postoperatively. Bleeding scores during tampon removal were lower in the levobupivacaine group compared to saline (p = 0.0016). Conclusion : The addition of 0.25% levobupivacaine to nasal tampons after FESS significantly reduces postoperative pain without causing hemodynamic disturbances.
