Abstract

Background: Dry Eye Disease (DED) is a common complication after cataract surgery due to tear film instability, ocular surface inflammation, and reduced reflex lacrimation. Phacoemulsification, the standard cataract surgery technique, may affect ocular surface integrity; thus, evaluation of tear film changes postoperatively is essential. This study aims to assess changes in DED parameters (OSDI, Schirmer I, TBUT, Ferning) and analyze tear film recovery time following phacoemulsification. Methods: This prospective clinical study involved 45 cataract patients who underwent phacoemulsification. Examinations were performed preoperatively, and at postoperative day 1, week 1, and week 4. Parameters included Ocular Surface Disease Index (OSDI), Schirmer I test, Tear Break-Up Time (TBUT), and Ferning test. Data were analyzed using paired t-test, Wilcoxon test, and Kaplan-Meier survival analysis. Results: OSDI scores increased significantly from 23.49 ± 7.576 to 30.02 ± 7.526 on day 1, remaining above baseline at week 4 (26.00 ± 7.323; p < 0.05). Schirmer I decreased from 11.00 ± 3.336 mm to 7.00 ± 2.036 mm on day 1 and only recovered to 10.00 ± 2.060 mm at week 4. TBUT declined from 11.00 ± 3.231 seconds to 8.00 ± 2.421 seconds on day 1 but returned to baseline by week 4. Ferning scores increased from 2.00 to 3.00 at week 1 and normalized by week 4. Kaplan-Meier analysis showed the fastest recovery in OSDI, TBUT, and Ferning (median 1 week), while Schirmer I required longer recovery (median 4 weeks). Conclusion: Phacoemulsification induces temporary deterioration in DED parameters. TBUT and Ferning recovered within 4 weeks, whereas Schirmer I and OSDI required longer recovery, emphasizing the need for close monitoring and supportive therapy after surgery.
