Abstract

Background: Convergence insufficiency (Cl) is a binocular vision disorder characterized by difficulty maintaining adequate convergence when viewing near objects, leading to symptoms such as eyestrain and diplopia. Common therapeutic approaches include home-based pencil push-up (PPT), virtual reality-based (VR) vision therapy, and a combination of both. This study aimed to compare the effectiveness of these three methods in improving the symptoms and clinical signs of CI. Methods: This randomized controlled trial with a single-blind design involved 36 medical students diagnosed with Cl, who were equally assigned to three treatment groups: PPT, VR, and combined therapy. Interventions were conducted over 12 weeks. Outcome measures included the Convergence Insufficiency Symptom Survey (CISS) score, Alternate Cover Test (ACT), and Near Point of Convergence (NPC), assessed at baseline, and at weeks 4, 8, and 12. Data were analyzed using paired and independent /-tests with a significance level of p<0.05. Results: All three therapies produced significant improvements in CISS and NPC scores (p<0.001). Significant improvement in ACT values was observed in the VR and combined groups (p<0.01), but not in the PPT group (p>0.05). Among groups, the combined therapy demonstrated the greatest reduction in mean CISS score (from 25.5 to 16.7) and improvement in NPC (from 12.9 cm to 8.9 cm), followed by the VR group (CISS decreased from 24.5 to 18.6). All between-group comparisons at week 12 showed p<0.01. Conclusion: All three vision therapy modalities effectively improved symptoms and convergence function in CI. However, the combined therapy yielded the most consistent and optimal results, suggesting a synergistic effect between ocular motor exercises and immersive YR-based visual stimulation.
