Abstract
Dengue infection can manifest as dengue fever (DF) and dengue hemorrhagic fever (DHF). Plasma leakage finding, such as; haemoconcentration, pleural effusion, ascites, hypoalbuminemia or shock, differ DHF to DF. This study would analyze the difference of pleural effusion index (PEI), level of haemoconcentration and hypoalbuminemia in every severity degree of DHF. A cross sectional study was done from October 2008 until March 2009 in Moh. Hoesin Hospital Palembang. The severity of DHF was classified into DHF without shock (grade I and II) and DHF with shock (grade III and IV). Serial hematocrite and platelet examination, albumin concentration, ultrasonography and right lateral decubitus chest x-ray was performed in all patients. Hypoalbuminemia was classified into mild (albumin ≥ 3 mg/dl) and severe ( albumin < 3 mg/dl). Pleural effusion was classified into mild (PEI≤ 10) and severe (PEI>10). Data were analyzed using chis-quare test or Fisher’s exact test and the correlation using pearson correlation. A P value less than 0.05 was considered significant. This study identified 245 patients with suspected dengue infection; i.e. 150 (61.2%) DHF; 68 (37.8%) DF and 27 (11%) other febrile illness (OFI). Ninety five DHF patients were eligible in this study; 31 (32.6%) grade I, 18 (18.9%) grade II, 39 (41.1%) grade III and 7 (7.4%) grade IV. Mean haemoconcentration percentage, albumin concentration and PEI in DHF without shock and with shock are; 27.2±9.1 % and 35.9±10.1%; 3.0±0.4 g/dl and 2.7±0.5 g/dl; 7.5±1.1 and 12.7±2.5 respectively. Severe hypoalbuminemia, higher haemoconcentration percentage, and severe pleural effusion are significant finding in DHF with shock (p=0,001); (p=0,003); (p=0,000) respectively. Weak correlation is found between pleural effusion and haemoconcentration percentage (r=0,228;p=0,023), and hypoalbuminemia (r=0,327; p=0,001). Severe pleural effusion, higher haemoconcentration percentage and severe hypoalbuminemia are common finding in DHF with shock. Correlation between pleural effusion and haemoconcentration, and hypoalbuminemia in DHF patient is weak.
