Abstract

A high ratio of references in Puskesmas Muara Pinang exceed ordinance standards Social Security Agency 15%, with the highest disease diagnosis referenced included 144 diseases that must be completed at the health center so that the effect on improving customer service at the hospital. The aim of this study was to analyze the implementation of the referral system participants in the National Health Insurance Puskesmas Muara Pinang 2019. This study used qualitative methods. The information was collected through in-depth interviews to 11 informants of Puskesmas Muara Pinang, Chairman BPJS Empat Lawang and Patient Health JKN participants, as well as the observations and analysis of documents. Analysis of the data used is content analysis. Test validation through triangulation of sources, methods and data are then processed and described based on the results of coding. The results of the study that the understanding of capitation-based commitments of Puskesmas Muara Pinang is good, on the implementation of three main indicators have not reached the target of assessment is BPJS, understanding health centers as a gatekeeper has been good, the availability of doctors at Puskesmas Muara Pinang is not good, financing ambulance referral borne by BPJS with procedures for filing a claim, the implementation of the policy aspect in Puskesmas Muara Pinang is not good, the reason most patients ask for a referral is based on the desire itself, the implementation of referrals in Puskesmas Muara Pinang already implementing the referral system on line. It was concluded that the implementation of the referral system in Puskesmas Muara Pinang has not been run in accordance with prescribed rules seen from the high ratio exceeds the reference standard provisions BPJS is 15%. Suggestions in this study is BPJS Health should review all aspects of policy regarding the implementation of a referral system and can disseminate to the entire community.
