Abstract

Background: In young age, women is at higher risk for stroke than men. Pregnancy and puerperium may contribute to the increased risk of stroke in young women. Relative risk of ischemic stroke in the postpartum period is 8,7. Hypercoagulable state during pregnancy along with complications such as eclampsia and HELLP syndrome (Hemolysis, Elevated Liver enzymes, Low Platelet) which cause vasospasm and thrombotic microangiopathy could cause cerebral ischemia and infarction in women during their pregnancy and delivery period. Case report: A 16 years old woman hospitalized for postpartum eclampsia and partial HELLP syndrom experienced spastic left-sided hemiparesis (arm strength 0, leg strength 4-), left supranuclear VII and XII nerve palsy, and left-sided hemihypesthesia three days after delivery. Head CT scan showed infarction located in right temporoparietal lobes. Patient was treated with MgSO4 in accordance with eclampsia protocol, dexamethasone for HELLP syndrome and methyldopa, followed by aspilet and neuroprotector. She also underwent physiotherapy. There was no abnormality noticed in echocardiography and carotid doppler, however transcranial doppler revealed low MFV and PI which was a sign of right MCA occlusion. She was discharged with motor improvement (arm strength 2, leg strength 4). Head MRA one month after stroke onset revealed stenosis at M1 segment of right MCA, her arm strength was 4 and leg strength was 5 at that time. Conclusion: This report is about ischemic stroke in a young woman with postpartum eclampsia and partial HELLP syndrome who had motor improvement after treatment. Stroke is an advanced complication in women suffered from eclampsia and HELLP syndrome during their pregnancy and delivery period which needs cautious attention and comprehensive treatment.
