Abstract

Pollution due to palm oil liquid waste that does not meet quality standards will have an impact on the balance of the environment and health. Palm oil waste liquid contains a lot of organic and inorganic substances. Wastewater Treatment Plant (WWTP) plays an important role in determining the quality of wastewater. The wastewater management system can also malfunction in operation, this will affect the quality of the wastewater produced. Therefore, it is necessary to analyze and identify potential system failure modes that have an impact on the environment. FMEA (Failure Mode and Impact Analysis) is a technique used to identify potential forms of failure risk, the impact of failure risk and provide corrective action as an effort to reduce system failure. This research is a descriptive study using qualitative methods that will be carried out at the identification stage, data collection, data processing, FMEA calculation table analysis and interpretation of the results. Based on the results of the analysis that has been carried out, the risk priority number that has the highest risk of failure in the treatment system is the circulation pump that is not functioning properly, so that there is no perfect dilution of wastewater. Meanwhile, the results of the analysis of the quality of wastewater show that the waste produced still does not meet the quality standards set, such as levels of BOD, COD, and oils and fats which are still quite high. However, for the quality value of pH and waste discharge still meet the quality standards. This potential risk of failure is caused by unscheduled maintenance activities and the difficulty of efforts made to dredge the husks in the WWTP pond. At the end of this study, corrective action was given by considering the hierarchy of control for any potential risks in the near future, including dredging and cleaning the husks in the WWTP pond that has been covered with husk, making a routine schedule for the operator on duty and making a budget plan to carry out pond WWTP maintenance periodically.
