Abstract

Placenta accreta is an abnormal attachment of the placenta to the uterine wall due to interference with endometrial decidualization, which is the process of thickening the uterine wall in preparation for implantation of the embryo. According to the level of trophoblast cells in the invasion, placenta accreta is divided into three types, placenta accreta, increta, and percreta. This study aims to determine the suitability of pelvic MRI images with the results of histopathological examination in placenta accreta patients at RSUP Dr. Mohammad Hoesin Palembang. This study is an observational analytic study with a cross-sectional study design. Samples were placenta accreta patients who met the inclusion and exclusion criteria and were taken using a non-probability sampling method by purposive sampling. The data used in this study is secondary data obtained through the medical record data at the Medical Record Installation, Radiology Department, and Anatomical Pathology Department, RSUP Dr. Mohammad Hoesin Palembang in January – October 2021. From 18 research samples, based on the sociodemographic characteristics of placenta accreta patients, the majority were in the maternal age group 20 – 34 years (66.7%), with the most occupation being a housewife (66.7%), mostly occurs in women with secondary level of education (88.9%) and multipara (83.3%) or a history of giving birth 2-5 times being the highest parity rate for placenta accreta patients. The most common surgical history found in placenta accreta patients was a history of CS ≥ 2 times (66.7%). Placenta previa totalis was the most common location for abnormal placental attachment (72.2%). Based on pelvic MRI images, placenta percreta was the invasion type with the highest prevalence (55.6%), whereas based on histopathological examination results, placenta accreta was the invasion type with the highest prevalence (44.4%). Statistically using the Kappa Cohen test, the level of reliability between the pelvic MRI images and the results of histopathological examination showed a minimal level of agreement (κ = 0.26)
