Abstract

Heart failure is a progressive clinical syndrome that occurs from changes in the structure or function of the heart that interfere with the ventricular ability to pump blood. Beta blockers are one of the main pharmacological therapy recommendation in stable heart failure. According to randomized trials, some beta blockers have been shown to reduce mortality in heart failure by inhibiting sympathetic system stimulation from β-adrenergic receptor in cardiac, thereby lowering blood pressure and heart rate. Another mechanism is about inhibition in RAAs to reduce diastolic pressure and postpone cardiac remodelling. However, there is still lack of data on the use of this drug in heart failure patients. The aim of this study was to determine the prevalence of the use of beta blockers in heart failure NYHA I and II at Dr. Mohammad Hoesin Palembang Hospital. The research used is a cross-sectional design in the form of a prevalence of drug use. This study was conducted for 1 month using medical record data of patients with heart failure in cardiac ward at Dr. Mohammad Hoesin Palembang hospital for the period November 1st, 2020 to October 31st, 2021 who met the inclusion criteria. There were 103 medical record data that met the inclusion criteria were processed using SPSS v26.The prevalence of beta blockers drug use was (71.8%) with the highest prevalence in men (62.2%), age 41-60 years (47.3%), and a history of hypertension (45.9%). In patients with NYHA I and II Heart Failure, the most beta blockers use is bisoprolol (100%) with the frequent dose was 2,5 mg (60.8%) and the highest frequency is 1 time/day (100%). The prevalence of beta blockers uses for heart failure NYHA I and II is quite high and it could be said that the usage of beta blockers was appropriate, according to PERKI. ACC/AHA, and ESC guidelines.
