Abstract

Background: Coronavirus disease 2019 is an infectious disease of the respiratory tract caused by a new type of coronavirus, namely Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). Patients with COVID-19 may experience impaired immune response that will cause hyperinflammation by the virus. Circulating biomarkers that can represent inflammatory status are potential predictors of the prognosis of COVID-19 patients, including; Neutrophil-to-Lymphocyte Ratio (NLR) and C-Reactive Protein (CRP). This study aims to compare the value of Neutrophil-to-Lymphocyte Ratio and C-Reactive Protein in ICU and non-ICU COVID-19 patients at Dr. RSUP. Mohammad Hoesin Palembang. Method: This research was classified as an analytic observational study with a cross sectional design. The sample used is secondary datas in the form of medical records and laboratory results of COVID-19 patients at the Medical Record Installation and Central Laboratory of Dr. RSUP. Mohammad Hoesin Palembang in the period of January 2021 - June 2021 who met the inclusion and exclusion criteria using a consecutive sampling technique. Result: In this study, 240 COVID-19 patients were determined as research subjects. There were 32 patients (13,3%) in the ICU and 208 patients (86,7%) in the non-ICU. The median age of ICU patients was 56 years, most were in the age range ≥ 65 years, that is 11 patients (34,4%), the sex ratio between women and men was the same, with 1 most comorbid, that is 5 patients (38,5%) with type 2 DM and 2 patients (15,4%) with hypertension. The median NLR for ICU patients was 12,75 and the median CRP was 102,5 mg/L. The median age of non-ICU patients was 52 years, most were in the age range of 45 - 54 years, that is 48 patients (23,1%), the sex ratio between women and men was the same, most without comorbidities followed by 1 most comorbid, that is 19 patients (28,8%) with hypertension and 15 patients (22,7%) with malignancy. The median NLR of non-ICU patients was 3,295 and the median CRP was 28 mg/L, with 66 patients (31,7%) having CRP < 5. Conclusion: The comparison of NLR and CRP values was significant between the two groups of COVID-19 patients in ICU and non-ICU (p=0.000).
