Abstract

Irritable Bowel Syndrome (IBS) is a functional gastrointestinal disorder and commonly present in children and adolescences, with prevalence in Indonesia raise to 32,2%. The pathophysiological mechanisms of IBS are unclear, and still challenging to determine organic disorders, A community-based survey with comparative cross sectional approach from 6 high schools in Palembang. Subjects were recruited using the multistage random sampling divided into two groups (70 subjects IBS and 70 subjects nonIBS). The Rome III criteria are used to establish a diagnosis of IBS along with a questionnaire to determine risk factors. Analyzed for association with Blastocystis hominis infestation, intestinal mucosal integrity. Direct microscopic stool examination to identify single Blastocystis infection was performed. Examination of antitrypsin alpha-1 and fecal calprotectin levels by ELISA to determined impaired intestinal mucosal integrity. Of the 454 subjects, the prevalence of IBS was 30.2%. The major risk factors for IBS were bullying, girls, ages 14–16 years, history of constipation, three nuts, beverages, and history of diarrhea (range OR 2.86–1.81). As 454 subjects with an IBS prevalence of 30.2%, with the main risk factors for IBS being bullied, girls, ages 14-16 years, previous illness of constipation, three nuts, beverages, and previous illness of diarrhea (range OR 2.86–1, 81). The results of the analysis of 140 subjects found the probability of IBS among adolescences if had previous illness of diarrhea, previous illness of constipation, Blastocystis hominis infestation, and inflammation gut mucosal, three nuts (peanuts / cashews / almonds), beverages, at 98.60%. There were 2 models. Model 1 is a model that can be applied to primary health services with sensitivity, specificity of 78.1%, 51.1%, aiming as a screening test for the occurrence of IBS in adolescents. Model 2 is intended for tertiary health services, with sensitivity, specificity of 74.3%, 86.6%, respectively, as management of juvenile IBS. Model 1 and Model 2 have a cutoff point ≥ 4 based on the calculation of probability and prediction of the score. As 36 subjects with Blastocystis hominis infestation were administered with metronidazole 3 x 500 mg for 10 days showed significant improvement. The final result from this research is a leaflet form as counseling media and early detection for IBS among adolescents. Conclusion: From predictive score analysis, Model 1 is intended as IBS filter test for adolescents in the community / UKS and Model 2 as the basis for managing IBS in adolescents in Indonesia with fairly good sensitivity and specificity.
