Abstract

Chronic renal failure (CKD) is a condition in which the kidneys are progressively and irreversibly damaged, resulting in failure to maintain metabolic, fluid, electrolyte and acid-base balance and have increased urea creatinine and decreased glomerular filtration rate due to exacerbation of nephritis, urinary tract obstruction, kidney damage. vascular disease due to systemic disease (diabetes mellitus, hypertension) and form scar tissue in the blood vessels. The main causes of kidney failure are diabetes mellitus and hypertension, both of which are the main triggers of chronic kidney disease in Indonesia. Chronic kidney failure was the 27th leading cause of death in the world in 1990 and increased to 18th in 2010. The management of chronic kidney failure is kidney transplantation or temporary kidney therapy such as hemodialysis peritoneal dialysis (PD). Hemodialysis is usually done 2 times a week, each hemodialysis ranges from 3-5 hours or an average of 4 hours each time hemodialysis is performed. This activity continues throughout his life. Patients with chronic kidney disease often experience depression and it is not uncommon for problems suffered by a person to affect their psyche, such as feeling afraid, worried, confused, stressed, and not confident. One of the effects of this condition is that the patient becomes a lot of stress (stress) so that it will disrupt the sleep cycle and even sleep disturbances. One of the non-pharmacological interventions to overcome sleep problems is progressive muscle relaxation therapy. Progressive muscle relaxation therapy can focus on a muscle activity by identifying tense muscles and then reducing tension by performing relaxation techniques to get a relaxed feeling. Applying nursing care in cases of chronic kidney failure and sleep management to improve sleep quality according to evidence based. The method used is descriptive qualitative method with a case study approach. The results of the study obtained nursing diagnoses, namely 4 actual diagnoses, 1 potential diagnosis and 1 risk diagnosis. Interventions carried out to overcome sleep problems are providing sleep support and relaxation therapy (progressive muscle relaxation therapy). Implications of progressive muscle relaxation therapy after being given for 3 days can improve sleep quality from low sleep quality category to adequate sleep quality and high sleep quality. Progressive muscle relaxation therapy has several stages, namely before starting therapy, first creating a quiet and comfortable room, limiting external stimuli, encouraging the patient to relax and focusing, then being given a PQSI questionnaire. Nursing care was carried out on 3 managed patients which consisted of a process of assessment, diagnosis, intervention, implementation, and evaluation. Progressive muscle relaxation therapy has an effect on improving sleep quality in CKD patients.
