Abstract

According to data from visit and referral of Public Health Office in Palembang 2020, from 20.043 visits in Pakjo Primary health Center Palembang,4878 patiens were referred to the referral ration at 24,33%. Examined from the diagnoses of diseases that are often referred to, these diseases are included in the 144 diagnoses of diseases that must be completed at the Puskesmas according to the Health Minister Regulation of the Republic of Indonesia Number 5 of 2014. The purpose of this study was to analyze implementation of referral health care policy of National Health Insurance participants in Pakjo Primary Health Center Palembang. This study was a qualitative study. The data was collected by indepth interview to 9 informants. Triangulation was done by sources, methods, and data. The results of the research on the aspects of standards and objectives are known that the standard used is 15% and aims to reduce the reference ratio. In terms of resources, doctors are always at the Puskesmas according to working hours, information technology has been used to support the recording and reporting of referral services, and the availability of medical equipment is inadequate and the availability of drugs is limited. In the aspect of the implementing organization, namely the SOP in the implementation of referrals based on the Health Minister Regulation of the Republic of Indonesia Number 1 of 2012 and Health Minister Regulation of the Republic of Indonesia Number 5 of 2014, for engagement with related parties needs to be increased. Aspects of disposition, doctors have supported the Minister of Health RI No. 5 of 2014 although the demand for patient referrals is still quite high. In the aspect of communication, namely the communication that has been done is quite good. In the aspect of the social environment, it is known that the patient's attitude influences doctors to give referrals and there are still mistakes in the misuse of bureaucratic structures in the political environment. It was concluded that of the six aspects studied, there were two aspects, namely the standard and objective aspects and the communication aspects which had been implemented well, while the other four aspects were still not good. The Pakjo Health Center is expected to evaluate and monitor the availability of medical equipment and medicines, update the SOP for referral services and add internet providers.
