Abstract

Service Commitment-Based Capitation is a capitation payment system from BPJS Health to FKTP based on the fulfillment or achievement of four indicators that are applied as an effort to improve the service quality of JKN participants in FKTP. Kutaraya Health Center is a puskesmas that has implemented a Service Commitment-Based Capitation (KBKP) system. The purpose of this study was to examine efforts to achieve indicators of Contact Number (AK), Outpatient Referral Ratio for Non-Specialized Cases (RRNS), and the Ratio of Participants in Routine Visits to FKTP (RPPB) in the implementation of KBKP (Capitation Based on Service Commitment) at the Kutaraya Health Center, District Ogan Komering Ilir.This study uses a qualitative method with a case study research design. The number of informants is 1 key informant, 4 main informants and 6 additional informants. With each informant including key informants consisting of the person in charge of P-Care. The main informants consisted of the 2020 JKN Treasurer, the 2021 JKN Treasurer, Doctors, and P-Care Operators. Additional informants consisted of JKN participants for AK indicators, referrals and prolanis. Data analysis used the interactive analysis model proposed by Miles & Huberman which consisted of 3 components, namely data reduction, data presentation, and verification. The results of the study indicate that the implementation of the Service Commitment-Based Capitation system at the Kutaraya Health Center has not been fully implemented properly. Based on the four indicators of Service Commitment-Based Capitation, there are 3 indicators that are below the safe zone target, including the contact number indicator, the ratio of non-specialist case referrals and also prolanis. The number of contacts is due to the large number of participants registered at the puskesmas and also the perception of the community. The referral ratio is caused by the lack of understanding and knowledge of the community. Prolanis was caused because it had not been implemented since the implementation of the KBKP system. It was concluded that the implementation of the Service Commitment-Based Capitation system at the Kutaraya Health Center so far has not been optimal. Because of the three KBKP indicators, all indicators have not met the target. It is recommended for the puskesmas to work together with BPJS health to provide training to the teams involved in efforts to achieve the KBKP indicators
