Abstract

Background: Patient with history of previous cesarean section in the next delivery can either repeat cesarean section or vaginal birth. Data about predictive factor, maternal and fetal outcome has not been reported and still controversy. This research was aimed to identify maternal and fetal characteristic in vaginal birth after cesarean and repeat cesarean section at RSUP dr. Mohammad Hoesin period January 2012-Desember 2012. Method: This research was observational descriptive study using cross-sectional approach. Total population were 1586 with 168 patients in inclusion criteria. Data were obtained through medical records which divided into three ways of delivery (elective repeat cesarean section, emergency repeat cesarean section and vaginal birth after cesarean). Results: From 168 samples, distribution of elective repeat cesarean section was 54.8%, emergency repeat cesarean section was 22% and vaginal birth after cesarean was 232%. The most common gestational and maternal age, birth weight and class of each group was aterm, 20-35 years old, 2500-4000, and 3"1 class. The highest indication of elective repeat cesarean section was social indication and emergency cesarean section was premature rupture of membranes. Duration of maternal and newbom hospital stay was greater in repeat cesarean section group. The most frequent maternal complication is in emergency cesarean section group. Perinatal death and severe asphyxia was greater in vaginal birth after cesarean. Conclusion: The most common distribution of gestational and maternal age, birth weight, and class were same in each ways of delivery. Complication and duration of hospital stay was greater in emergency repeat cesarean section. Perinatal death and severe asphyxia more common in vaginal birth after cesarean, this influenced by any other factor like maternal and fetal condition.
