Abstract

Background: Cholesteatoma is an important complication of COM which can cause high morbidity. Cholesteatoma has a tendency to enlarge progressively and erode the bone structure of the middle ear, it can even spread from outside the middle ear to the brain causing life-threatening complications. Although most cholesteatoma can be diagnosed by otoscopy, the clinician still cannot determine the size and extent of the lesions. Therefore, CT scan plays an important role as an evaluation tool before surgery which is the only treatment option for COM patients with cholesteatoma because the imaging results can alert the otologists to the danger during surgery. Thus the otologists can choose safe and effective surgical procedures, so that the condition of postoperative patients also gets better. Method: This study is an analitic observational study using a cross-sectional research design. Data of this research was obtained through secondary data. Data was analyzed using SPSS version 24.0 and Epicalc. Results: In this research, 47 COM with cholesteatoma patients included. Most the samples were men (53,2%), and were 26-35 years old (31,9%). Sensitivity, specificity, PPV, NPV and accuracy of the auditory bone are 78%, 90%, 91%, 75% and 83%. Sensitivity, specificity, PPV, NPV and accuracy of mastoid air cells are 100%, 75%, 92%, 100% and 94%. Sensitivity, specificity, PPV, NPV and accuracy values are 100%, 75%, 92%, 100% and 94%. Conclusions: CT Scan has a good prediction value on evaluating preoperative COM patients with cholesteatoma.
