Abstract

Introduction: Hyperuricemia is a condition where uric acid levels are above normal levels (> 7 mg/dL in women and > 6 mg/dL in men) and can cause negative impact on a person's body if left untreated. Long term hyperuricemia will lead to gout arthritis, kidney stones and increase risk factor of hyperlipidemia, diabetes, hypertension and other cardiovascular diseases. So far, allopurinol is the first drug of choice used to reduce uric acid. Allopurinol has structure that is similar to xanthine, two aromatic rings accompanied by hydroxyl group which can inhibit the xanthine oxidase enzyme and metabolized it into alloxanthin. Just like allopurinol, cherry plants (Mutingia calabura L.) have been shown in previous studies to reduce uric acid because they contain flavonoid compounds which also have two aromatic rings accompanied by hydroxyl groups. The parasite of the cherry plant (Dendrophthoe pentandra (L.) Miq), was found to contain the flavonoid quercetin and other secondary metabolites similar to its host. Therefore, research is needed regarding the xanthine oxidase enzyme inhibitory ability of the ethyl acetate polar fraction and extract of cherry mistletoe leaves. Methods: The simplicia of cherry parasite leaves was extracted by maceration method using ethyl acetate solvent. The results of the ethyl acetate extract of cherry mistletoe leaves were fractionated using ethanol and ethyl acetate solvents in 5 types of ratios, namely F1 (9:1), F2 (7:3), F3 (5:5), F4 (3:7) and F5 (1:9). In concentration 75; 150; 300; 600; and 1200 ppm, ethyl acetate polar fraction and extract of cherry mistletoe leaves then carried out phytochemical tests and xanthine oxidase enzyme inhibition tests. The percentage value of enzyme inhibition at 5 types of concentrations in each sample will be load in linear regression analysis to obtain IC50. Results: Ethyl acetate polar fraction and extract of cherry parasite leaves contain alkaloids, flavonoids, steroids, terpenoids, and tannins. The IC50 value of the extract and polar fraction (F1 – F5) ethyl acetate of cherry mistletoe leaves was classified as very active and sequentially the results of the extract and polar fraction (F1 – F5) ethyl acetate of cherry mistletoe leaves were 9.86 ppm; 1.38 ppm; 1.14 ppm; 6.19 ppm; 9.41 ppm and 5.90 ppm. The ability to inhibit the xanthine oxidase enzyme is very active due to the content of metabolites found in the cherry parasite plant. Flavonoids, alkaloids, tannins and terpenoids in cherry parasite leaves are able to prevent the formation of uric acid by binding to molybdenum so that xanthine fails to attach to the active site of the enzyme. Conclusion: There are secondary metabolites of alkaloids, flavonoids, steroids, terpenoids, and tannins in the ethyl acetate polar fraction and extract of cherry parasite leaves which can inhibit the xanthine oxidase enzyme and have a very active IC50.
