Abstract

Introduction : Erythema nodosum leprosum (ENL) also known as type 2 leprosy reaction is a systemic acute inflammation reaction that is found in 30-50% of all leprosy patients. Leprosy patients with type 2 leprosy reaction is at risk of developing permanent damage or even disabilities. To this point, the diagnosis of ENL depends only on clinical observation and tends to be delayed. ENL is an acute inflammation reaction that is closely related to components of immune system such as neutrophils, lymphocytes, and monocytes. Immunologic parameters like neutrophil-to-lymphocyte ratio (NLR) can be considered as an alternative for early marker for ENL in leprosy patients. This study aims to identify the association of neutrophil-to-lymphocyte ratio (NLR) and the occurrence of erythema nodosum leprosum (ENL) in leprosy patient. Method : This study is an observational analytic with cross-sectional design that uses the data obtained from medical records of leprosy patient seen between January 2017 and June 2022. Inclusion and exclusion criteria is used to select the eligible patients for this study. Result : Among a total of 87 leprosy patients in this study, 28 of them (32,2%) were reported with ENL. The median of NLR is 3,55 with the value of cut-off point for identifying ENL is 3,3. NLR and ENL showed a significant association with the P value of 0,001. The prevalence ratio (PR) is 2,015. Conclusion : Neutrophil-to-lymphocyte ratio (NLR) can be considered as an early biomarker for erythema nodosum leprosum (ENL) in leprosy patients which is proven by a significant association reported in this study.
