Abstract

Background: Respiratory failure is a condition where there is a significant failure of gas exchange capacity in the respiratory system or a syndrome due to failure of the respiratory system to carry out oxygenation or elimination of carbon dioxide. Cases of respiratory failure are indications for patients to be treated in the Intensive Care Unit (ICU), and mechanical ventilation is a means of assisting oxygen in patients with respiratory failure in the ICU. The main problem that occurs in patients with mechanical ventilator connected to endotracheal tube connector or tracheostomy tube is obstruction of the airway. Effort to maintain airway patency was namely by suction intervention. Objective: Describes the implementation of nursing practice that is focused on the implication of suction in nursing care and evidence based practice in patients with respiratory failure with mechanical ventilation. Method: Descriptive in the form of case studies. Results: The results of the study found that the three managed patients had respiratory failure, decreased consciousness after craniotomy surgery, and were connected to SIMV mechanical ventilation via tracheostomy. There are 7 nursing problems that are raised, the problems experienced by the three managed patients are ineffective airway clearance and ineffective breathing patterns. The suction intervention given affects the change in the patient's oxygen saturation value. Discussion: Suction is a nursing intervention performed by inserting a suction catheter tube through the nose, mouth, endotracheal tube, tracheostomy tube which aims to clear the airway, reduce sputum retention, stimulate cough reflex and prevent lung infection. Suction intervention is carried out repeatedly and periodically according to the patient's condition, using the open and closed suction method. Conclusion: Suction intervention has an effect on changes in oxygen saturation values that increase, the patients are not anxious, and looks comfortable in breathing. So that suction intervention needs to be continued until the problem is resolved.
