Abstract
Oral surgery procedure, like tooth extraction is the common procedure in dentistry: Adequate perioperative medication is very important to maintain this operative procedure. The metaholism, excretion of many drugs and their pharmacologically active metabolites depend on normal renal function. This will be a problem when w e face patients with renal failure, as w e know that renal failure itself can injluence perioperative dental medication’s ejfectivity. Renal failure patient is more susceptible of infection, bleeding, anemia, and drug toxicity, so we have to notice the appropriate drug selection and dosing adjustment in renal failure patient. The nephrotoxic agent like mefenamic acid' tetracycline, penicillin must be excluded from dosing adjustment in renal patient, and if it is possible we can change with non nephrotoxic drug, example clyndamicyn, acethaminophen, or opioid. Ifit is not possible to exchange the nephrotoxic drug, we can adjust the appropriate dose for renal failure patient. A familiarity with basic pharmacologic principles and a systematic approach are necessary when adjusting drug dosages in patient with renal failure. If done properly, therapy in renal patient should achieve desired pharmacologic ejfects while avoiding drug toxicity.
