Abstract

Introduction. Upper gastrointestinal bleeding (UGB) is a life-threatening condition in children which require immediate treatment. Endoscopy as the gold standard intervention for this case is expensive, invasive, and not always available in medical facility. In adults with UGB, scoring system to accurately predict the need for endoscopy exist. In children, there is only one scoring system. It is Sheffield Scoring System. However, the ability of this score to identify the need for endoscopic intervention has not been validated widely. Method. A diagnostic accuracy study comparing the Sheffield score with the endoscopy results to assess its sensitivity and specificity to predict the need for endoscopic intervention occurred in Dr. Mohammad Hoesin General Hospital from July 2022 – February 2023. There were 31 subjects with UGB who underwent esophagogastroduodenoscopy and for whom the Sheffield score was calculated. Results. Based on research, the majority of subjects are female. The subjects were dominated by the age group > 10 years, the majority came from Palembang and carried out initial admissions through outpatient department. The main complaint of most subjects is hematemesis. The specificity of Sheffield's score on 10 cutoff is 81.8%, specificity is 85.0%, positive prediction value is 75.0%, negative prediction value is 89.4%, and accuracy is 83.8%. Conclusions. The sensitivity, specificity, and accuracy of Sheffield Scoring System are good. This score can be used in Indonesia to predict the need for endoscopic intervention in children with UGB.
