Abstract

The maternal mortality rate has also increased since the establishment of Coronavirus Diseases 19 (COVID-19) as a pandemic. This has resulted in many social restrictions and restrictions on almost all health services including maternal and neonatal services both in the Independent Practicing Midwife (BPM), Community Health Centers and Hospitals. According to POGI data until April 2021 there were 536 pregnant women in Indonesia who were exposed to COVID-19, of these 16 people died or around every 1000 pregnant women, 32 of them died. Whereas in the city of Palembang in 2020 there were 59 deaths per 100,000 live births, 4 of which were maternal deaths, so it is necessary to analyze the implementation of ANC services during the COVID19 pandemic in Palembang City. The purpose of this study was to analyze the implementation of Antenatal Care (ANC) policies in an effort to reduce maternal mortality during the COVID-19 pandemic in Palembang City. This research method uses qualitative methods by collecting observational data and in-depth interviews with 23 informants who have the main duties and responsibilities that influence the implementation of ANC policies at the Puskesmas. Data validation was carried out by method triangulation, source triangulation, and data triangulation and then the data were analyzed using thematic analysis. The results showed that the Puskesmas coordinated and communicated with the City and Provincial Health Offices, but in terms of service transmission it was not optimal. In terms of resources, training and supporting facilities for ANC services, such as Hazmat PPE, are still lacking. The disposition aspect of HR implements policies by holding commitments. The role of the bureaucratic structure is that SOPs at the Puskesmas already exist and follow policies from the center but there are still health workers who are negligent in implementing SOPs. The conclusion of this study is that the implementation of ANC policies during the Pandemic period in Palembang City has not been implemented optimally and there are still several obstacles in the implementation of policies that cause ANC services to not be optimal so that they affect the Maternal Mortality Rate. Policy makers need to make regulations or rules for implementers and policy makers regarding how to disseminate and enforce regulations, monitor, evaluate the implementation of ANC so that it can be carried out optimally. The results of this study are expected to be used as material for monitoring and evaluation to increase the coverage of ANC visits and services. Keywords: Anatenatal Care, COVID-19, Pregnant Women, Maternal Mortality Rate Angka kematian ibu juga meningkat sejak ditetapkannya Coronavirus Diseases 19 (COVID-19) sebagai pandemi, Hal tersebut membuat banyak pembatasan sosial dan pembatasan hampir ke semua layanan kesehatan termasuk pelayanan maternal dan neonatal baik di Bidan Praktek Mandiri (BPM), Puskesmas maupun Rumah Sakit. Menurut data POGI hingga bulan April 2021 ada 536 ibu hamil di Indonesia yang terpapar COVID-19, dari jumlah tersebut ada 16 orang meninggal dunia atau sekitar setiap 1000 ibu hamil, 32 diantaranya meninggal. Sedangkan di kota Palembang pada tahun 2020 ada 59 kematian per 100.000 kelahiran hidup, 4 diantaranya merupakan kematian maternal sehingga perlu dianalisis implementasi pelayanan ANC di masa pandemi COVID-19 di Kota Palembang. Tujuan penelitian ini untuk menganalisis implementasi kebijakan Antenatal Care (ANC) dalam upaya penurunan angka kematian ibu dimasa pandemi COVID-19 di Kota Palembang. Metode penelitian ini menggunakan metode kualitatif dengan pengambilan data observasi dan wawancara mendalam dengan informan 23 orang yang memiliki tugas dan kewajiban utama yang berpengaruh dalam mengimplementasikan kebijakan ANC di Puskesmas. Validasi data dilakukan dengan triangulasi metode, triangulasi sumber, dan triangulasi data kemudian data dianalisis menggunakan analisa tematik. Hasil penelitian menunjukan Puskesmas berkoordinasi dan berkomunikasi dengan Dinas Kesehatan Kota dan Provinsi namun dari segi transmisi pelayanan dilakukan tidak optimal. Dari segi sumberdaya masih kurang maksimalnya pelatihan dan fasilitas pendukung pelayanan ANC seperti APD Hazmat yang masih terbatas. Segi disposisi SDM melaksanakan kebijakan dengan memegang komitmen. Peran struktur birokrasi bahwa SOP di Puskesmas sudah ada dan mengikuti kebijakan dari pusat namun masih ada petugas Kesehatan yang lalai dalam penerapan SOP. Kesimpulan penelitian ini implementasi kebijakan ANC pada masa Pandemi di Kota Palembang belum dilaksanakan secara optimal dan masih ada beberapa kendala dalam pelaksanaan kebijakan yang menyebabkan pelayanan ANC tidak maksimal sehingga berpengaruh pada Angka Kematian Ibu. Pemangku kebijakan perlu membuat regulasi atau aturan bagi pelaksana maupun dari pembuat kebijakan terkait cara mensosialisasikan dan menegakan regulasi, monitoring, evaluasi pelaksanaan ANC agar dapat terlaksana dengan optimal. Hasil penelitian ini diharapkan dapat dijadikan bahan monitoring dan evaluasi untuk meningkatkan cakupan kunjungan dan pelayanan ANC.
