Abstract

Background. Microcytic anemia with the highest prevalence in Indonesia is iron deficiency anemia (IDA). IDA leads to impaired children development and vulnerable to infection. Gold Standard for diagnosis EDA is serum ferritin examination. This examination not widely available and expensive therefore required more convenient, cheaper, faster, and safer examination. Hameed and Mentzer index are accurate index and has been used for screening IDA. This study aimed to compare Hameed and Mentzer index accuracy and to know which one is more accurate. Methods. This study is a diagnostic test with cross-sectional design. Subject of the study were collected from September 2015-2016 at RSUP Dr. Mohammad Hoesin Palembang. 106 microcytic anemia patients aged 1-18 years who enrolled in this study, the hematology data taken for Hameed and Mentzer index calculation. The results of index calculation compared with serum ferritin levels to calculate the value of validity IDA screening index. Hameed and Mentzer index accuracy differences were analyzed with McNemar difference test. Results. From the results of receiver operating characteristic (ROC) curve analysis, cut-off point Hameed index 6.64 and Mentzer index 15.92. By using the cut-off point, Hameed index showed sensitivity 88%, specificity 86%, accuracy 87%, and Youden index 74. Mentzer index sensitivity 96%, specificity 72%, accuracy 80%, and Youden index 68. McNemar test results show the value of p=0.022. Conclusion. Hameed and Mentzer index modification accurate for screening of iron deficiency anemia in pediatric patients at RSUP Dr. Mohammad Hoesin Palembang. However, the Hameed index accuracy is better than Mentzer index and both of index significantly different.
