Abstract
Background: Instrumentation during root canal treatment will produce a smear layer. The irrigation functions to remove debris, pulp, microorganisms, smear layer, and necrotic tissue. Anatomical variations make the apical third of the root canal difficult to clean. The sonic agitation technique is effective in cleaning the smear layer in the apical third due to its large amplitude. NaOCl is an irrigant that is widely used because it has broad-spectrum antimicrobial properties, and can dissolve necrotic pulp and tissue, but can cause pain and inflammation if extruded periapically. An alternative irrigation material that can be used is ClO2 because it has broad-spectrum antimicrobial properties, low toxicity, and can dissolve organic and inorganic tissue. Objective: To determine the effectiveness of 0.3% ClO2 and the effectiveness of irrigation using the sonic agitation technique on the cleanliness of the smear layer in the apical third of the root canal. Methods: This in-vitro quasi-laboratory experimental study used 30 permanent mandibular premolars divided into 2 groups. Samples were prepared using the crown-down technique. The samples were irrigated according to the treatment group, namely group A was irrigated using 0.3% ClO2 with a sonic agitation technique, and group B was irrigated using 2.5% NaOCl with a sonic agitation technique. The samples were cut mesio-distally and the root canal cleanliness was assessed using a measuring microscope. The root canal cleanliness score was based on the cleanliness scale of Torabinejad et al. Data were analyzed using the Kappa statistical test, Kolmogorov-Smirnov test, and Mann-Whitney test. Result: 0.3% ClO2 with the sonic agitation technique had a lower average cleanliness than 2.5% NaOCl with the sonic agitation technique, but the difference between the two was not significant (p = 0.051). Conclusion: 0.3% ClO2 solution with sonic agitation technique is effective in cleaning the apical third of the root canal.
