Abstract

Background: Cesarean section is the most commonly performed major operation worldwide. Based on Riskesdas 2018 data, the prevalence of sectio caesarea in childbirth in Indonesia is 17.6%. Patients with a history of cesarean section mostly underwent repeated cesarean sections at subsequent births. Recurrent cesarean sections are known to have higher operative difficulty and longer operative time. Therefore, there is speculation that there will be differences in pain control during the postoperative period of primary and repeated cesarean section. This study is expected to compare postoperative pain between the two types of patients who use the same type of anesthesia with the hope of reducing the morbidity and mortality associated with cesarean section. Methods: This study was conducted using an analytic observational method with a cross sectional design, using primary data from patients underwent sectio caesarea at RSUP Dr. Mohammad Hoesin Palembang with consecutive sampling technique. Results: The subjects consisted of 84 primary and 84 repeated caesarean section patients. In the Wilcoxon signed rank test analysis, it was found that the mean VAS score between the 3 period studied had a significant difference (p value = 0.000 for both patient groups). In the Mann Whitney analysis, there was a significant comparison of the median VAS score between the 3 period studied (p value = 0.000 for both patient groups). Conclusions: There is a comparison between the frequency of cesarean section with the perception of pain felt by the subject. The repeated cesarean section group experienced a higher level of pain than the primary cesarean section group. Keyword: “Postoperative pain for cesarean section, primary cesarean section, repeated cesarean section, spinal anesthesia, VAS score.
