Abstract

Latar Belakang: Neuropati optik toksik etambutol adalah gangguan pada saraf optik akibat efek samping dari penggunaan obat etambutol pada pasien tuberkulosis. TB-MDR (Tuberculosis Multi Drug Resistant) adalah suatu keadaan dimana kuman M. tuberculosis sudah resisten dengan kedua OAT lini pertama yaitu isoniazid dan rifampisin. Penelitian ini bertujuan untuk mengetahui prevalensi dan hubungan faktor resiko kejadian neuropati optik toksik etambutol pada pasien TB-MDR. Metode: Penelitian ini merupakan observasional analitik dengan desain cross-sectional. Sampel penelitian ini adalah data rekam medik pasien TB-MDR yang mendapatkan terapi etambutol dan dikonsulkan ke bagian mata di RSMH Palembang. Data di analisis dengan uji Chi-Square untuk mengetahui hubungan antar variabel dan analisis regresi logistik untuk menentukan faktor risiko yang paling berpengaruh terhadap kejadian neuropati optik toksik etambutol. Hasil: Dari 12 pasien TB-MDR yang mengalami neuropati optik toksik etambutol, ditemukan kebanyakan pasien mengeluhkan penurunan sensitivitas kontras (50,0%) dan memiliki faktor risiko yaitu berusia >60 tahun (p=0,003; OR=6,556; 95% CI=1,732-24,820), dosis etambutol <1200 mg/hari (p=0,389) dan durasi ≥6 bulan (p=0,126; OR=3,293; 95% CI=0,669-16,211), riwayat gangguan fungsi ginjal (p=0,746; OR=1,455; 95% CI=0,148 – 14,260) dan riwayat DM (p=0,160; OR=2,400; 95% CI=0,690-8,344). Kesimpulan: Secara statistik terdapat hubungan yang signifikan antara faktor risiko Usia lanjut dengan kejadian neuropati optik toksik etambutol. Secara statistik tidak terdapat hubungan yang signifikan antara faktor risiko dosis dan durasi penggunaan etambutol, riwayat gangguan fungsi ginjal dan riwayat DM terhadap kejadian neuropati optik toksik etambutol. Faktor risiko yang paling berpengaruh terhadap kejadian neuropati optik toksik etambutol yaitu faktor risiko usia lanjut, setelah dikontrol oleh variabel confounding.
