Abstract

Tuberculosis infection has a risk 16-27 times greater that occurs in people with HIV / AIDS. The presence of infection can interfere with the body's immune response in HIV patients and the development of HIV AIDS infection so that HIV patients with infection pose a greater risk of dying. The purpose of this study was to determine the predictors of the incidence of TB-HIV coinfection in Raden Mattaher Jambi Hospital from 2016-2020. This study uses an analytic observational study design with a case-control method (1:2). The sampling technique used is purposive sampling with a total sample of 180 patients consisting of 60 cases and 120 controls. Data was collected using ART register data obtained from the VCT clinic and medical record data. Data were analyzed by univariate, bivariate (chi-square) and multivariate (logistic regression). The results showed that the variables were clinical stage [OR= 13 (4,862-34,756)], CD4 levels [OR= 11,484 (5,408-24,388)], anemia status [OR= 6,668 (3,345-13,293)] and opportunistic infections [OR= 5,714 (2,842-11,488)] showed a significant relationship to the incidence of TB-HIV coinfection, while the variables gender [OR= 0,789 (0,422-1,433)], education level [OR= 0,874 (0,423-1.80,)], employment status [OR= 0,755 (0,380-1,503)] and marital status [OR= 0,920 (0,458-1,846)] did not have a significant relationship with the incidence of TB-HIV coinfection. In multivariate analysis, CD4 level was the most risk factor for TB-HIV coinfection. Patients with CD4 levels less than 200 cells/mm3 had a 3.9 times greater risk for TB-HIV coinfection after controlling for anemia status, opportunistic infections and clinical stage variables [AOR= 3.928 (1.463-10.547)]. Patients with CD4 levels less than 200 cells/mm3 should pay more attention to their health conditions by routinely performing ART therapy, taking ARV drugs, routinely checking CD4 levels and checking for tuberculosis in accordance with the recommendations of health workers.
