Abstract

Background: Hepatitis B is an infectious disease caused by the hepatitis B virus. This virus replicates in hepatocyte cells which have the potential to be acute or chronic. Persistent or chronic hepatitis B infection is characterized by persistent HBsAg in the body for more than 6 months. Scar tissue due to injury in the liver determines the progression from fibrosis to liver cirrhosis. The suppression of viral replication using antiviral therapy in the form of nucleoside analogs for the long term can improve liver inflammation and fibrosis as well as stop progression to liver cirrhosis and HCC. The most common non-invasive method for diagnosing liver fibrosis in chronic hepatitis B disease is transient elastography/TE (fibroscan). Fibroscan is also known as transient elastography and it has the advantage in diagnosing liver fibrosis up to stage 4. Methods: This research is an analytic observational study with a retrospective longitudinal design using repeated measurements. The subjects were 32 chronic hepatitis B patients who received nucleoside therapy and met the inclusion criteria. Data were obtained from secondary data and then they were analyzed using Wilcoxon. Results: After being observed in the last 1 year (12±24 months) of therapy, the fibrosis degree before therapy was dominated by F4 (cirrhosis) (65,6%) and it changed to (25,0%) after the therapy. The mean liver stiffness at the beginning of the therapy was 18,17 ± 10,21 kPa and it became 10,91 ± 7,97 after the therapy. The results of the Wilcoxcon.showed a significant difference (p = 0,000), and a significant decrease in the levels of SGPT, SGOT and HBV DNA (p < 0,05). Conclusion: There are changes in the fibrosis degree before and after nucleoside therapy with a significant decrease in the stage of fibrosis and liver stiffness based on fibroscan.
