Abstract

Latar Belakang dan Tujuan. Sepsis adalah suatu kondisi yang ditandai oleh systemic inflammatory respon syndrorne (S1RS) dan proses infeksi yang telah dikonfirmasi. Penelitian Harrison 2006 melaporkan adanya peningkatan persentase insiden sepsis dalam kurun waktu 1996-2004. Penelitian Angkasekwinai 2007 melaporkan bahwa mortalitas karena sepsis mencapai 34,3%. Upaya yang dapat dilakukan untuk mengatasi permasalahan kompleks tentang sepsis yaitu memahami patogenesis dari sepsis itu sendiri, salah satunya mengenai respon imunitas berlebihan yang disebabkan oleh polimorfisme. Oleh karena itu, penelitian ini dilakukan untuk mengidentifikasi polimorfisme promoter -308 G/A gen tumor necrosis factor a pada penderita Sepsis di RS. Mohammad Hoesin. Metode. Penelitian ini merupakan penelitian deskriptif terhadap 48 sampel penderita sepsis. Identifikasi polimorfisme promoter -308 G/A gen tumor necrosis factor a ini dilakukan dengan teknik Polimerase Chain Reaction (PCR) dan Restriction Fragmen Length Polymorphism (RFLP) menggunakan enzim Ncol. Hasil. Genotip GA/AA (polimorfisme) 20,8 % dan Genotip GG (wild type) 79,2%. Berdasarkan sosiodemografi, frekuensi genotipe GA/AA (polimorfisme/ terbesar pada rentang usia 21-30 (6,3%), mayoritas (12,5%) pada jenis kelamin laki-laki, lokasi infeksi primer utama pada traktus digestivus (8,3%), dan berasal dari Banyuasin (12,5%). Sebagian besar (62,5%) penderita sepsis adalah laki-laki, rentang usia 41-50 adalah rentang usia penderita sepsis terbanyak (22,9%), traktus digestivus (45,8%) merupakan lokasi infeksi primer tersering, dan mayoritas (31,2%) penderita sepsis berasal dari palembang. Kesimpulan. Pada penderita sepsis di RSUP dr. Mohammad Hoesin Palembang yang dijadikan subjek penelitian, didapatkan hasil sebanyak 10 orang (20,8%) yang mengalami polimorfisme heterozigot dan homozigot (mutan). Sedangkan 38 orang lainnya (79,2%) tidak terbukti mengalami polimorfisme (wild type).
