Abstract

Background: Variations in drug response may lead to treatment failure and/or adverse effects on individuals and subpopulations. Genetic factors have the greatest influence as the cause of this variation. The enzyme cytochrome P450 2A6 (CYP2A6) is an enzyme involved in xenobiotics metabolism phase in liver cells. CYP2A6 plays a role in the metabolism of coumarin, nicotine, and nitrosamine. In vitro studies with human liver microsomes and determination of phenotype in vivo shows the variation of the individual’s CYP2A6 activity. Phenotype studies show the human population is divided into groups of extensive metabolizer (EM) and poor metabolizer (PM) associated polymorphisms in the CYP2A6 gene. Wild-type allele will provide EM phenotype, whereas the variant allele CYP2A6*2 and CYP2A6*3 provides PM. This study aims to reveal the genotype and allele gene variant CYP2A6*2 and CYP2A6*3 of CYP2A6 gene on four major ethnic of Palembang population, that is: ethnic Malay, Chinese, Arab, and Indian population in Palembang. Method: This study is a descriptive observational approach to 69 participants that consist of 4 major ethnic populations in Palembang. The data were taken are the primary data and confirmation of ethnic was done by filling the pedigree form of three generations. Identification of CYP2A6 polymorphisms was conducted by single-step PCR (Polymerase Chain Reaction) amplification and continued by RFLP (Restriction Fragment Length Polymorphism) detection using Xcm\ enzyme for variant alleles CYP2A6*2 and DdeI enzyme for the detection of variant alleles CYP2A6*3. Result: The distribution of genotype CYP2A6*1/*1 on participants were founded 69/69 (100%) in the four ethnic groups. The ffequency of genotype CYP2A6*l/*2, *l/*3, *2/*2, *2/*3, *3/*3 in all four ethnics are 0/69 (0%). Frequencies of CYP2A6*1 allele founded 69/69 (100%), CYP2A6*2 0/69 (0%) and CYP2A6*3 0/69 (0%). Conclusion: Allele variant CYP2A6*2 and CYP2A6*3 mutation was not found in the study participants' ethnic Malay, Chinese, Arab, and Indian of Palembang population.
