Abstract

Background. Diabetes mellitus (DM) is a group of metabolic diseases characterized by chronic hyperglycemia caused by defects in insulin secretion, insulin action or both. Type 1 DM generally occurs in childhood. Symptoms and clinical signs are the same as those of DM in general, including: polyuria, polyphagia, polydipsia, and weight loss. More than 1/3 cases of type 1 DM in children are diagnosed early in diabetic ketoacidosis (DKA). Improper management of children with DKA can result in cerebral edema or kidney failure resulting in death. The purpose of this study was to look at the profile of type 1 DM in children at the time of initial diagnosis and the current profile. Method. The research conducted was descriptive observational research. Data obtained from secondary data, the medical records of RSUP Dr. Mohammad Hoesin Palembang and primary data in the form of filling out questionnaires. Results. There were 70 children with type 1 DM at RSUP Dr. Mohammad Hoesin Palembang for the 2019–2021 period. There were 39/70 patients (55.7%) with female sex, the most age group was 11–15 years, 30/70 patients (42.9%) with a median age of 11 years. 36/70 patients had good nutrition (56.3%) and 18/70 patients (25.7%) had HbA1c levels >8–9% in the last 3 months. As many as 37/70 patients (52.8%) had DKA after being diagnosed with type 1 DM. The average insulin dose was 0.7–1.0 IU/kgbb/day. The age of initial diagnosis of type 1 DM ranged from 7.56 ± 4.053 years, the smallest age was 2 years, with the most age group being 6–10 years as many as 28/70 patients (40%). 37/70 patients (52.9%) had no family history of DM. Polyuria occurred in 61/70 patients (87.1%), weight loss occurred in 51/70 patients (72,9%) and 37/70 patients (52.8%) presented with DKA. Patients came with referrals 55/70 (78.6%). C-peptide levels in 15/16 patients <0.2 nmol/l (93.8%) and only 7 patients with HbA1c levels <9%, 63/70 patients (90%) with HbA1c levels >9%. Conclusion. Initial diagnosis in type 1 DM patients, 61/70 (87%) patients came with polyuria, 37/70 patients came with DKA and 63/70 patients had poor glycemic control (HbA1c > 9%). The age range when first diagnosed was 6–10 years (40%).
