The data retrieval tool with the action test is assessed through the observation sheet which refers to the instrument that has been compiled and then tested to calculate the validity and reliability. The result of data analysis at each successive cycle shows that there is an increase from cycle I to cycle II. Evident from the percentage results in each cycle has increased in a row.
