Abstract

Malaria menjadi salah satu masalah kesehatan dunia yang mempengaruhi angka kematian. Dalam mencegah dan membasmi penularan penyakit Malaria, dilakukan upaya kegiatan mengamati data dan informasi secara berkala atau yang bias disebut dengan kegiatan Surveilana Malaria. Kegiatan tersebut dilakukan sebagai upaya deteksi dini dalam mencegah terjadinya Kejadian Luar Biasa atau KLB. Dalam kegiatan surveilans malaria ditemukan permasalahan pada proses pencatatan dan pelaporan yang dilaksanakan oleh petugas. Pencatatan secara Pa�per based yang dapat mengakibatkan catatan tidak terstruktur dan rentan hilang, Pengumpulan laporan yang berbasis Spread Sheet pada Microsoft Excel membuat petugas kesulitan karena memiliki banyak jumlah baris data. Proses pada penelitian ini, menerapkan metode Design Thinking guna memperoleh kebutuhan dan keinginan pengguna pada protorype aplikasi. Dalam merancang formulir, penulis memanfatkan penerapan Form Design Pattern agar dapat memberikan solusi berupa formulir yang menyenangkan dalam penggunaannya. Hasil dari penelitian ini berupa perancangan atau prototype yang telah diuji dan mendapatkan hasil berupa prototype yang dirancang sesuai kebutuhan serta pengguna merasa puas dan dapat diterima oleh pengguna.
