Abstract
Chronic renal failure is a condition in which kidney function decreases progressively and can cause various complications that can increase the potential for Drug Related Problems (DRPs). The purpose of this study was to determine the incidence of Drug Related Problems (DRPs) and the percentage of incidence of DRPs in patients with chronic renal failure with or without complications at RSUD Siti Fatimah South Sumatra Province during the October-November period in 2023. This study is a non-experimental descriptive study with prospective sampling of 44 medical record data of chronic renal failure patients who meet the inclusion criteria. The inclusion criteria were inpatients suffering from chronic renal failure with or without stage 5 complications and undergoing hemodialysis, patients with an age range ≥ 25 years and complete and legible medical records. Types of DRPs were analyzed using the PCNE classification method, the use of the PCNE classification, is important to separate real and potential problems related to DRPs. The analysis of the results showed that Drug Related Problems (DRPs) that occurred in patients with chronic renal failure were drug interactions (40,75%) and polypharmacy (59,25%). Chi-square analysis showed that the number of drugs significantly increased the incidence of potential Drug Related Problems (DRPs) in patients (p<0.05). The more drugs used will affect the incidence of DRPs. Key words: Chronic renal failure, DRPs, PCNE, Prospective
