Abstract

Background: Shaping process in root canal treatment will produce smear layer. The use of mechanical instrumentation with irrigation solution can reach areas that are difficult to clean, especially in the apical third due to normal variations. One of the irrigation technique that can be used is manual agitation. This technique uses a gutaperca with a frequency of 100 push and pull movements for 30 seconds to produce a hydrodynamic effect. Chlorine dioxide irrigation solution can dissolve organic and inorganic tissue (smear layer). Aims: To determine the effectiveness of ClO2 0.1% and 0.3% on the cleanliness of the smear layer in the apical third using manual agitation technique. Methods: This quasi-experiment used 28 mandibular premolars and divided into 4 groups. Samples were prepared with the crown down technique and irrigated according to the treatment group: (A) ClO2 0.1% (B) ClO2 0.3% (C) NaOCl 2.5% with EDTA 17% (D) distilled water. The sample was split into two parts so the total sample was 56 and observed using measuring microscope at 1000× magnification. The observation results were analyzed using Kappa test, Kruskal Wallis test, and post hoc Mann Whitney test. Results: There was a significant difference in smear layer cleanliness in the apical third between groups A-B, A-C, B-D, and C-D, but no significant difference between groups A-D and B-C. Conclusion: Chlorine dioxide (ClO2) with a concentration of 0.3% using manual agitation technique is effective in cleaning the smear layer in the apical third of the root canal.
