Abstract

The WHO reports that 95% of all maternal deaths occur in low- and middle-income countries. In 2021, antenatal care (ANC) visit coverage in Indonesia reached 88.13%. However, 17 provinces have not reached the target, including West Papua and Papua which have coverage below 40%. Susenas 2023 data shows that 76.04% of women of reproductive age (WUS) (15-49 years old) have health insurance, with details of 44.56% having BPJS Kesehatan PBI, 25.28% BPJS Kesehatan Non-PBI, 2.58% company or office insurance, and 0.54% private insurance. There were still 23.96% of WUS who did not have health insurance. This study aims to analyze the relationship between health insurance ownership and the utilization of antenatal care services among women in Indonesia. This study used secondary data from the 2017 IDHS with a cross-sectional research design. The study sample consisted of 4,720 respondents who met the inclusion and exclusion criteria. Data analysis included univariate, bivariate, and multivariate analysis. The results showed that 83.8% of respondents made ANC visits ≥ 6 times. Bivariate analysis showed a significant association between age, mother's education, employment status, parity, residence, economic status, exposure to information media, health insurance ownership, husband's education, husband's support, and desired pregnancy with ANC service utilization. Multivariate analysis showed that health insurance ownership had a significant association with ANC service utilization (p-value < 0.0001; PR = 1.248; 95% CI = 1.111-1.403) after controlling for the variables of parity, place of residence, economic status, husband's education, husband's support, and desired pregnancy. It is recommended that the Indonesian Ministry of Health work with the Ministry of Religious Affairs to add a marriage requirement of having health insurance to cover the cost of the antenatal care they receive.
