Abstract

Introduction: Diabetes Mellitus (DM) type 2 is a chronic disease characterized by increased blood sugar levels due to inadequate insulin production in the body. One of the nursing problems that must be addressed quickly to avoid more serious complications is ineffective peripheral perfusion. Buerger Allen Exercise (BAE) is a therapy that is carried out with various variations of active postural movements in the plantar area by applying gravity regularly. Purpose: To describe the implementation of nursing care for type 2 DM patients and the application of BAE. Methods: Case study with 3 type 2 DM patients in the RSMH Palembang inpatient ward. Results: The assessment data on the three patients showed that the patient complained of tingling and numbness, CRT 4 seconds, the acral felt cold, the skin color was pale, the skin turgor decreased, the patient tired easily, the patient was often thirsty and had frequent urination, the results of the last GDS examination in the three patients were >200 mg/ dl, the patient complained of nausea, and his vision was blurry. The nursing diagnoses for the three patients included 4 actual diagnoses with nursing problems of ineffective peripheral perfusion, instability of blood glucose levels, fatigue, and nausea, as well as 1 risk diagnosis with nursing problems of risk of falls. Discuss: The main interventions provided are hyperglycemia management, circulation care, peripheral sensation management, activity/rest education, energy management, nausea management, and fall prevention as well as BAE application support interventions. Implementation is a dependent, independent and interdependent action. The BAE application is given for 10-15 minutes for 3 days. Conclusion: After being given nursing care for 3 days, all nursing problems were resolved. BAE application can be done independently by the patient because it is effective in improving peripheral circulation of the lower extremities, is economical, and has no side effects.
