Abstract

ABSTRACT Background: Elevated estradiol indicated poor outcome. Aim of this study is to determine the accuracy of estradiol level as predictor of 28-days mortality for patients with sepsis in ICU. Metode: This study is an observational cohort prospective analytical study. Patients were diagnosed with sepsis based on the SOFA score. Blood samples were taken from the vein and estradiol levels were measured in the laboratory. The patients were observed for 28 days. Patients who survived until the 29th day but were still treated in the ICU or moved to ward before 28 days of treatment were considered as the survivor group. Result: From 23 patients, the number of patients alive after 28 days was 15 patients (65.2%) and patients who died were 8 patients (34.8%). Sepsis patients who died had estradiol levels with an average of 57.38 ± 9.23, higher than patients who lived with an average of 39.73 ± 5.85 with p value = 0.038. The cut-off value of estradiol level was > 30 as a predictor of mortality in sepsis patients with an area under the curve (AUC) of 0.767 (p = 0.008), sensitivity of 87.5% and specificity of 66.7%. Discussion: Estradiol had a significant difference in mortality in sepsis patients. Sepsis patients who died had higher estradiol levels than living patients. Blood estradiol concentrations in sepsis patients did not differ in men and women. Conclusion: Estradiol levels with a cut-off >30 can be used as a predictor of 28-day mortality in sepsis patients.
