Abstract
Background: Gonorrhoea is the second most common sexually transmitted infection after Chlamydia trachomatis in developing countries. Female sex workers (FSWs) are considered the "core group transmitters," disseminating the infection to their clients and, subsequently, to the broader population. Several studies have reported resistance of Neisseria gonorrhoeae to antibiotics such as ceftriaxone, gentamicin, azithromycin, cefixime, and kanamycin. Antibiotic resistance in N. gonorrhoeae poses a significant public health threat, as it may result in increased disease transmission, higher morbidity rates, and escalated treatment costs. Therefore, routine surveillance of gonorrhoea prevalence and periodic antibiotic susceptibility testing are recommended. The findings of this study are hopefully to provide a foundation for the refinement or maintenance of existing therapeutic guidelines. Objective: To determine the incidence rate of gonorrhoea in FSWs at Sukarami Public Health Centre in Palembang and determine the susceptibility of N. gonorrhoeae to ceftriaxone, gentamicin, azithromycin, cefixime, and kanamycin. Methods: A descriptive analytical laboratory observational study with a cross-sectional design, conducted from May to October 2024 at the Sukarami Public Health Centre in Palembang and key population hotspots within the Sukarami Public Health Centre’s service area. A consecutive sampling was applied to recruit a total of 43 samples. Endocervical specimens were collected from each subject for the identification of N. gonorrhoeae. Antibiotic susceptibility testing was performed using agar dilution, the gold standard method. Results: Ten out of 43 study subjects (23.25%) tested positive for gonorrhoea based on Gram stain, culture, and bacterial identification tests. All N. gonorrhoeae isolates were sensitive to ceftriaxone. However, 10% of the N. gonorrhoeae isolates exhibited resistance to azithromycin and cefixime, 50% were resistant to gentamicin, and 60% were resistant to kanamycin. Conclusion: No resistance to ceftriaxone was observed in N. gonorrhoeae isolates; however, resistance to gentamicin, azithromycin, cefixime, and kanamycin was detected among FSWs with gonorrhoea at the Sukarami Health Centre in Palembang. Key words: susceptibility, Neisseria gonorrhoeae, ceftriaxone, gentamicin, azithromycin, cefixime, kanamycin.
